Hometown Family Healthcare, PLLC is a Nurse Practitioner owned

Family Medical Clinic located in the heart

of Chelsea in the historic Rexal Building.

Cynthia McNitt,
APRN-CNP

Cynthia has been an RN since 1999. She has worked in various hospital settings during the course of her career with the majority of her time spent in labor and delivery or nursery/NICU. Cynthia graduated in 2018 from Graceland University with her Master's of Science in Nursing degree and is a board certified Family Nurse Practitioner through the American Academy

of Nurse Practitioners. Since graduation, she has been providing primary

care in Chelsea. Cynthia takes pride in listening to her patients’ concerns

and treating them as a whole person. She

is invested in each and every one of her patients and treats them like

family. Cynthia is a hometown girl. She was born and raised in Pryor and now

lives in the Adair area with her husband and daughters. They have a

cattle ranch with an array of other animals.

Her free time is spent with family and friends, serving with her church,

enjoying her yearly girls’ beach trips, and going on adventures in her

Jeep.

Rachel Young,
APRN-CNP

Rachel’s experience in the medical field began at the age of 13 when she started volunteering at a nursing home with her grandmother. This

experience led her to pursue a career in healthcare. Rachel began

her career as a Respiratory Therapist and later went to the

University of Oklahoma in Tulsa to pursue her Master’s of Nursing

Degree as a Nurse Practitioner. She is also board certified through

the American Academy of Nurse Practitioners. As a Nurse Practitioner

student, she did several clinical rotations here at Hometown Family

Healthcare and says she has felt “at home” since. Her goal as a provider is

to make each patient feel individualized and to enhance each patient’s

quality of life.

Rachel is the mother of three beautiful children, which she will tell you is

her proudest and the biggest accomplishment of her life. Hometown

Family Healthcare is thrilled to have such an amazing provider with our

practice.

Dr. Bubbles

Bubbles has passed away, We will always miss her and her sweet, tender heart. She loved being at the office and she loved all her patients. RIP 2010-2024

Tater Tot, Dogtor in Training

There's a new kid in town. Tater Tot has joined the Hometown crew. He is currently in training. He brings a new level of energy to the office.
